5 Whys: get to the root cause of a problem

The 5 Whys method means asking “why?” several times in a row, starting from an observed problem, until you reach a cause you can act on. Born at Toyota, it stops you from fixing symptoms that will come back.

When to use it

How to do it, step by step

  1. Describe the problem factually (who, what, when).
  2. Ask “why did this happen?” and write down the answer.
  3. Ask “why?” again about that answer, and repeat.
  4. Stop when the cause is actionable (often after 3 to 5 whys, not a strict rule).
  5. Define a corrective action for that cause and check later that it worked.

Example

Problem: the delivery is one day late. Why? Testing failed late. Why? It only ran the day before. Why? Nobody was in charge of scheduling it. Action: add a scheduled test task to the sprint.
